Student Nursing Survey

We value your feedback and encourage you to complete this survey so we can gauge what is most important to our student nurses.

Vaccine Mandate

As you are likely aware, the U.S. Supreme Court upheld the ability of CMS to require organizations who receive Medicare and Medicaid reimbursement to implement a mandatory vaccine policy. This policy extends to all individuals working in covered healthcare settings, including students. IRMC is implementing our COVID vaccine policy in response to this decision. As a result:

Effective January 27, 2022 – all students completing clinical rotations or internships at IRMC or affiliated entities must have the first vaccination or a pending request for a medical or religious exemption.

Effective February 28, 2022 - all students completing clinical rotations or internships at IRMC or affiliated entities must have completed the primary vaccination series or have an approved request for a medical or religious exemption.

For vaccinated individuals, documentation of vaccination must be provided to our Employee Health Department. For large group clinical rotations, faculty should compile a list of all students and provide documentation with one submission. For individuals completing rotations or internships, documentation will be requested as part of the pre-field experience clearance process. Students who are also employees do not need to complete the form twice.

Religious Exemption Form

Medical Exemption: If you are submitting a medical exemption above, you must also print the Medical Certification for COVID-19 Vaccine Exemption form for your physician to complete. Once completed, please send to Employee Health via email at vclevenger@indianarmc.org.
