IRMC Social Services has a team-based approach to medical care. Our experienced social workers collaborate with physicians, medical staffs, administrators and various agencies to facilitate comprehensive medical care for patients in the Indiana, Pennsylvania, region.
Our social workers address medical needs for patients who are experiencing mental health conditions, chronic health conditions, addiction, environmental barriers, and transitioning from a higher level of care back into the community.
Our team assist patients with:
- Creating health plans for depression and anxiety.
- Crisis management.
- Transition to a higher level of medical care.
- Addressing substance abuse and reviewing treatment options.
- Referrals to community agencies.
- Assessing pregnancy needs.
- Helping uninsured patients connect to medical assistance programs.
- Brief intervention.
After Care Program
The IRMC After Care program was designed for patients who visit the emergency department. Within a few days of your emergency department visit, a registered nurse (RN) will reach out to you to see how you are feeling and whether follow-up care with a primary care provider or a specialist is necessary. The RN will address any issues related to medication, assist you in coordinating your healthcare and schedule any follow-up appointments.
M.O.M.S. (Maternal Opioid Medication Support)
M.O.M.S. Perinatal Program is for expecting mothers struggling with addiction. You are not alone, we are here to support you during this important time. Our program is committed to taking care of you and your baby. Our partnership with Armstrong-Indiana-Clarion Drug and Alcohol Commission can provide you with treatment and resources for a healthy mom and baby. We understand addiction involves cycles of remission and relapse. Taking the steps for you and your baby can be a frightening decision. Remember, the benefits of treatment at any time during your pregnancy can improve a healthy outcome for both you and your baby.
